Amlodipine, Istin and Norvasc

Vascular mechanism

The US label describes reduced calcium entry into vascular smooth muscle, reducing vascular resistance; vascular effects exceed cardiac effects. Label pharmacology.

That biological mechanism and a clinical benefit estimate answer different questions. ALLHAT and CAMELOT: methods, funding and limits

ALLHAT randomised older, higher-risk people with hypertension to medicine strategies, including amlodipine and chlorthalidone, with additional treatment when needed. CAMELOT randomised people with coronary disease and normal blood pressure. Its composite included hospitalisation and revascularisation, rather than being a mortality-only outcome. Manufacturer money or drug supply places both trials in Tier 4/D; their benefit estimates are excluded from the independent conclusion.

An independence grade does not establish whether every study result is true. This review supplies neither a conflict-cleared benefit magnitude nor a ranking against every blood-pressure medicine. Swelling and urgent symptoms

Swelling, headache, dizziness, sleepiness, flushing and palpitations are reported effects. Severe persistent abdominal pain or blistering/peeling skin needs urgent assessment. Medicine effects.

SPS describes calcium-channel-blocker oedema as vasodilatory fluid pooling, rather than simply excess total-body fluid. It can affect both ankles. Tongue or throat swelling, difficulty breathing or swallowing, or collapse can indicate anaphylaxis: seek emergency help. Allergy warning signs.

Statins and other interactions

Valves, liver disease and feeding
